Has your Certificate of Operation expired?

Step 1: Getting Started: Contact a Certified Third Party State Inspector (We can help) and a Licensed Elevator Testing Company (Cardinal Elevator Services).

Step 2: Certificate of Inspection: To receive a Certificate of Inspection, the third party inspector must inspect the elevator with a Licensed Elevator Testing Company (Cardinal Elevator Services). Certified Third Party State Inspector will schedule with Cardinal Elevator Services for the elevator test, after the test is completed the Third Party State Inspector will submit the results of the inspection to the state.

Step 3: Inspection results: The Third Party Inspector will supply you with a report of the inspection results. If there are any deficiencies that need to be addressed, they will be provided. The Certified Third Party State Inspector will work with Cardinal Elevator Services to address any outstanding issues. After the state receives the inspection report, the state will send you an invoice for the Certificate of Operation. When the state receives payment of the invoice, the certificate will be issued and sent to you.
